What do you mean, you didn't "renew" it? Did you activly opt out of it? Or was it "open enrollment"? If you don't actively change something, it's still in force.

Does your employer collect in advance of the premium, or after the fact? You need to ask the EMPLOYER where that premium is, but it COULD have been paying for the prior month's insurance.
