Two separate questions in that one. All doctors must behave ethically, they give an oath. So, ethically, they cannot treat patients differently based on their insurance or worth.
First question - "would doctor create a pretend illness" - I would say "no way". There are corrupt doctors up there, but I find doctors to be one of the more ethical group of professionals.

Second question - "would better insurance or more money get you better care" - yes, I think it could. There are public hospitals up there, then there are private, better quality ones up there as well. Also, you can buy a care of a better doctor or some very expensive treatment plan that's not covered by general insurance and could not be afforded by people paying out of pocket. This has little to do with doctors, just economics. If you lived in a more socialistic country (and I was talking about US before) like Canada, Israel, Sweden, you could get much better care without having to pay extra.

The vast majority of group health insurance plans, the employer pays at LEAST 50%, otherwise, they don't get to write the insurance off their taxes. So it's pretty rare, that an employer pays some, but less than 50%.

It's not specific to YOU, so it doesn't violate the privacy laws. All they have to say is, we're from XYZ Health, determining spousal eligibility (don't even have to name the employee), can you confirm that you contribute less than 50% to the company health insurance plan. That's all they have to ask.

I would definitely make a claim. I don't know about you but for me $2,500. is a significant amount of money. And isn't that why you bought the jewelery coverage. Other than the typical annual rate change your rates will not be directly affected by this loss.

One word of caution if you have made more than one claim over the last two years you may want to check with your insurance agent because some companies will cancel you after a third loss. That is why you want to avoid claiming small losses. Don't submit small losses...

No. The dollars you use to pay your premiums comes from your employer who has already taken taxes out before you got the paycheck.

You are paying your premiums with after-tax dollars. If you want to pay with pretax dollars, you have to buy into your company's health plan.
