You seem to have covered all the bases and yet you have been denied twice for that elusive B2 visa. This is the deal. In the mind of the adjudicating consul, you do not have enough reasons to go back to the Philippines after your visit. They think you will marry in the US and just wait for your adjustment of status there. The only way you can go to the US is through a K-1/K-3 visa. No matter how many times you try, I do not think they will grant you a B2 especially now you have a record of 2 denials.

It might just be a swollen lymph node that got infected from either water, food or virus. I would wait and see what the doctor says, don't feel so worried right now.
