Before you switch, I really encourage you to phone the insurance company yoursef AND have your doctor SPECIFICALLY REQUEST that cymbalta be added to the formulary of covered medications based on "medical necessity" (the term is important) and file an appeal on your behalf.

Insurance companies that accept Medicare are REQUIRED to offer bariatric surgery to their enrollees regardless of plan type as long as you can demonstrate each of the following:

1) A body mass index over 35
2) At least one co-morbidity related to obesity
3) Documentation from your medical providers that your previous attempts at medical treatment for obesity have been unsuccessful

If you meet each of these requirements, Medicare currently requires insurers to cover three procedures: gastric bypass surgery, lap band surgery and duodenal switch surgery. You'll also need to choose a surgeon from the list of approved Medicare providers.

Before jumping out of your HMO, call your insurer and discuss this with them. You may not have to switch plans.

it does, but you have to make sure of two things.. one make sure that insurance doesn't fall off when they switch the ESN over.. you can do this four ways. ask the representative when they do it after to check to see if insurance is still on there.. check your very next bill and see if it stayed on there.. or ask them to mail your plan and services to you if it will be a while till your next bill.. the fourth is if you have my account you can check the next day under plans and services to see if it is still on there.

IF you find it fell off call immediately and have them put it back on, if it stays off for too long they cannot put it back on.. they will check to see if you requested insurance to be removed and if you didn't then they will reinstate it. IF you tell them, say you got a phone off ebay. then they have to remove insurance as Asurion (their insurance company) will not cover used phones without a verified past.. because your phone was on your account before it counts cause it's past is verified thqat it was eligible for insurance and has been covered before.
