I have never heard of this fee in my life. Doesn't sound right at all. Just out of curiousity where do you live? If it's Canada I can tell you for sure the fee is illegal, if it's elsewhere I am not sure. In Canada dentists charge by a government regulated fee guide, so they cannot tack on extras to cover costs. I am not sure what other countries do.

Generally claims have to be "sudden and accidental". It would appear to me that an insurance company would not cover this because they wouldn't view root growth as sudden. And collapse seems to be something that would happen over time as well. A drain or pipe bursting, however, would be sudden. I would turn in the claim, regardless and if they deny it have them site very specific reasons as to why.
