It means, bringing you back to the USA. Example: If you're on a cruise ship, and have a heart attack, you'll have to be flown back to the USA usually by plane from the nearest port's airport, or helicopter.

It can EASILY cost $50,000 for that.

Not exactly. Cigna has a foreign liability policy with a rider that a company can purchase, with a limited amount of "health" type coverage including emergency transportation via air to another country, for US residents that are in another (specified) country on business, on a short term basis. Not EVERY country is going to be included. It's not true "health" insurance, it just reimburses you usually up to $50,000, for emergency medical expenses - the broken arm, broken leg, heart attack kinda stuff. No meds are covered, and nothing is preauthorized. Oh, and preventative maintenance or long term conditions (such as pregnancy or cancer) are DEFINATLY not covered. It's really an accident coverage.
