Medicare doesn't cover surgery for solely cosmetic reasons so you would need to have a referral from a psychiatrist or other medical practitioner to the effect that it was required for some other reason. That wouldn't be hard to do for scars resulting from an accident.

You would need to ask your insurance carrier what they will require for documentation that this was not cosmetic in nature.

But even if you send this documentation they could deny the coverage after the fact. Pre-authorization is never a guarantee that it will be a covered benefit.

I don't know but spoil yourself with a holiday in India , They are 1/10 of the cost at home and they are very good Doctor. You can cruise the Web and find cost, Doctor everything before going.........I had a reverse vasectomy done in India for a little $250............They really great and do not drive expansive Mercedes Benz...........

It may be better off to go to you gyn and seek advice there. If Dr. decides that it's med. necessity ins. will cover it probably but you can expect to have fun getting them to realize that it's med. necessity, they probably will try to deny it few times. Good luck.

You will need to have a predetermination - the insurer will review your medical documentation and either approve or deny the request for surgery in advance.

Typically, to have a breast reduction covered, you'll need to document something similar to the following (these are one insurance company's actual requirements...most will be similar, but you'll need to find out exactly what your insurer's criteria are):

* Severe back, neck, or shoulder pain with documentation of at least 1 year of conservative medical attempts to treat (i.e. - support bra, medication, physical therapy, etc.)

* Documented need for removal of at least 400 grams of tissue per breast

* Other criteria such as skin ulceration, shoulder grooving, posture problems, etc.

Additionally, expect that your doctor will need to send photographs and medical records (including documentation of the year's worth of conservative treatment attempts).

You should be able to request your particular insurer's medical policy, so that you can review it with your doctor and determine whether or not you qualify.
