Don't respond to the spam messages saying they know some random company, click here . . . it is an invitation to computer virus and to identity theft.Back to your question, only 15 states mandate that fertility coverage to some level be provided and most do not cover IVF . . . of the ones that do cover IVF, they do NOT cover it in the situation of tubal ligation. The sentiment is that you made the decision to tie your tubes and your infertility is NOT due to a "medical condition" - therefore medical insurance will not cover it. At this point, it is also a pre-existing condition for a new insurance policy.
My recommendation would be to budget, save money, visit a fertility clinic to see if they have a payment plan, take out a loan, work a second part-time job, sell stuff on e-bay, see if your employer has a flexible spending account so that you can save money pre-tax for IVF. Is one tube blocked or is both tubes blocked? You only need one unobstructed tube to get pregnant.

In the United States - it is not about just the insurance company - it also depends on the state that you live in, where your employer is located etc . . . Currently 15 states in the US mandate some sort of fertility coverage. You can refer to the Resolve Website for info on the states that require coverage.http://www.resolve.org/site/PageServer?p…
Be warned even if your state required it - some businesses are headquarted in other states and offer their coverage through another state that may not offer benefits. If they are self-insured it is not covered, and you must meet their qualifications for infertility. The least expensive options are group policies usually through your employer or your partner's employer.
I also recommend that you contact Fertility Life lines - they have a free hotline that you can call and have someone help you to better understand your coverage and to do an analysis for you. They were great - gave them my policy info and they researched it, got me all of the info on what was and was not covered and explained it to me:

If you cannot find insurance coverage for fertility treatments here are some other suggestions:
1) Start with the RE - most doctors have a team that can help you with finding ways to pay for your treatment - they are amazing at figuring out your insurance benefits, coming up payment plans and may even be able to recommend medical studies / clinical trials that you may be able to participate in.
2) Grants -there are groups out there that fund raise for fertility treatments - you need to apply and cross your fingers.
3) Loans - banks and credit card companies (my friend used Capital One) offer fertility loans with fair interest rates and payment plans. Maybe see if you can borrow from a family member.
4) Serious budgeting - I also know couples who have taken part time / seasonal jobs to save up for treatments, sold things on ebay etc . . .

I only know of 1 grant but they require membership and only grant a few.There are shared risk programs and you still qualify at your age. You prepay for 3-4 treatments, if you don't get pregnant in the 3-4 treatments, you get your money back.
Capital One has a credit line "Family Fee Plan" for IVF patients.
There are studies always being done through IVF centers which ofter treatment at a much lower price.
Pharms like Freedom Drug offer free overnight shipping and discounts. Their cost of medicines are pretty low and it's a USA pharmacy.
I use to have a list of insurance companies that had fertility coverage but the coverages differ from state to state. Some states like CA require large companies to provide some type of fertility coverage and some cover IVF. This website has the insurance list and may still have the employer list which provides information on companies that have fertility insurance and what type of coverage they provide--IUI's and/or IVF.
Here's some insurance companies and financing plans that cover fertility treatment: AETNA, Beech Street, Blue Cross/Blue Shield (BC/BS), Cigna, United Healthcare, Humana, Great West, Healthchoice, and Private Healthcare Systems (PHCS).
