Personally, YES!!! I never travel outside of the country without it. Unfortunately, unless you need to actually use the insurance, it's money you won't see again. It can definately feel like you've taken $100 or so and just tossed it out the window, if your trip is uneventful and you don't need to use it.

However..... if, heaven forbid, something ever DID happen; you lose your luggage, you need to cancel your trip because of an emergency, something happens to you, like taking a nasty spell down a staircase, a slip or fall, get really sick, anything where you need immediate medical attention, or the need to be transported off the ship to a hospital, etc, you will be glad you had it! It is VERY expensive to be transported off of a ship and taken to a hospital! And can you always trust just any old hospital in a foreign country?

Yes, the economy hasn't been the greatest. We want to cut costs, save where we can. So some people skip the insurance, and they're fine. But it can be a game of Russian Roulette, where that one time you decided to skip it turns out to be the time when you needed it.

Safety and peace of mind is not something I advise cutting corners on. Also, shop around. Sometimes the travel insurance offered by the cruise company isn't as good as 3rd party companies. A good website is www.insuremytrip.com . Any insurance is better than none at all though.
