You will need evidence of visits to your doctor complaining of chronic back and shoulder pain.
Your insurance company will always want you to lose some weight, like 10 to 20 pounds depending on your size.
They wil not pay for a lift for cosmetic reasons but reductions are lifts.

She's going to have to go to a local high risk agent, and get quotes through them. Hopefully, she's not overweight, also - it's likely going to be very expensive if she's NOT overweight, and not available if she is obese (which is the case for most young type 2 diabetes people.)

She can talk to her own agent to get a referral to a high risk life guy.

It's likely going to cost 5 - 10 times as much as it would if she was healthy.

You'll want to visit a local independent agent to see what companies your BMI will allow you to qualify for, then the agent can search those companies to see if any cover the surgery. There may be a waiting period before they'll cover. Do not give up your current coverage before getting a policy in place.
