As far as the tax issue goes for not reporting income of child support, you can call the IRS fraud line w/ the children's names & SS#'s and report the added income that the mother receives. If you don't have their numbers, just give names, addresses or whatever you have & stress that 22K is an awful lot of income to not be reporting every year. It doesn't really help you out financially, but if nabbed, it might put the scare in her to wonder what next & maybe fly a little straighter. / Fiance can also apply to court for reduced child support based on medical hardship or reduction in employment not his fault. (They penalize for purposely staying under employed.)

There's lots you can do to decrease the cost.
Assuming that your a low risk pregnancy, get a midwife rather than an ob... you've just lowered your cost by a few grand right there. A midwife is trained & can do everything an ob can pretty much, she'll also know when things arnt going right & its time to call in for an ob... you'll be perfectly safe in the hands of a midwife.

Also consider a home birth. The midwife will attend & generally provide everything needed. I had a home birth for one of mine (I have 4) everything went great, and its so much nicer to be able to lay back in your own bed afterwards, rather than some strange hospital bed.

Total cost of an at home birth attended by midwife... approx $1000.
And my midwife took payments as long as the fee was paid by the time the babe was 6 weeks old, she didnt care
