Ida Said:

If I get a group insurance plan and add on weight loss surgery?

We Answered:

You cannot add on bariatric surgery as an option.

Most insurance companies have a choice of several plans from the basic up to the comprehensive for group policies. The surgery may be included on the comprehensive plans but it will depend on the company and your area.

The premium for any particular plan will depend on the size of the group and if the group is fewer than 20 it will also depend on the general health of the group. One person having a BMI great enough to be a candidate for the surgery may increase the premium for everyone in the group by $70 per month for a group of 15 or it could increase the premium by $300 for a group of 3.

We Answered:

In order to get group coverage you must have a minimum of four employees and have 75% participation to qualify.

Marcus Said:

does my employer who recieves a group rate for health insurance have to offer it to me?

We Answered:

Depends on how the employees are classed. If they offer it to all salaried employees, but not hourly employees, that's fine. Or they can offer it to all executive officers, or all board members, and that's fine, without offering it to non executives or non board members. It's very, very easy to divide employees into different classes - one you want to insure, and one you don't.

Obviously, you can't offer to some salaried without offering it to all, UNLESS you ALSO have a class of "board members" or such. The health insurance company is going to require a total list of employees, and a class of employee, to be sure that the employer is insuring (usually at least 75%) of the class they offer benefits to.

The OBVIOUS answer here, is to tell your boss, health insurance is an issue, and tell them you need group health insurance. And if he turns you down on it, then start shopping around for another job, where you WOULD have health benefits.

Arthur Said:

health insurance group - Do I have to give medical records?

We Answered:

Some insurance company's require each enrollee to fill out health questionnaire. It asks for age, medications, surgeries etc. Not a good idea to lie since in some cases they will request medical records from the doctor. They do this due to having to cover a pre-existing condition. And your rates my depend on the health of your employees. Its not a bad idea to have someone price out all of the policies in your area to get you the best rate. Also COSE may be able to help you as well.
