Simply don't go for the first price they quote you but spend a few days collecting data.

You tell them everything the ask for as they are interested only in what has happened in the last 5 years regardless of what has expired. The normally ask for the offence code, date of offence fine and ban if any - all of that will remain on your licence until at least 4 years from the date you committed the offence.
Whilst it is expired in terms of the DVLA totting up procedure if you fail to tell your insurer then they can make the insurance invalid in the event of a claim. When a claim is made they will want to see the licence

Yes it does appear on your credit report. I only found out recently... in January I was looking for car insurance and must have viewed about 30 different comparison sites. Last month I requested my credit report and low and behold, all the searches I had done were showing on the report. It takes a year for them to be removed (from your report). I called up the Credit Report company and asked for them to be removed, but they said it shouldn't "really" impact on any future credit I take out.

Normally, only moving violatiions (speeding, red light, etc.,) affect your rates, but not getting a required inspection could affect your eligibility for insurance; it would depend on the company. Bestter to put it down and not need it than to omit it and find out otherwise..

Yes, or you are in breach of contract, and possibly fraudulent, though you have taken nothing, but a cut in insurance. Time limits apply, usually 3 years, but you would probably know how long based on company's application. Surprisingly, they already might have this info through an interface or link with police, though I doubt it. Info is definitely available on your claims, sometimes erroneously!
