The other agency that you did not go thru must have uploaded the application without your signature and had it billed directly to them. They paid the premium for you and since you didn't pay it they turned you over for collection. You should have received a policy and multiple bills from that agency. What you need to do is take a copy of the policy you did have during that time to them and they will cancel it back to the date that they issued it. What you need to do is get a letter from them on their letterhead with an explanation of what happened. That collection is going to show up on your credit for years.
