as far as Medicaid...I have had a few friends who have been on it, and they say it covers EVERY THING....and as far as I know, private insurance you always have to pay something.

Allowed amount vary depending on where the provider is located. For instance, an Optometry provider in Los Angeles, probably has a much bigger allowed amount with Insurers, than say an Optemetry Clinic in the middle of Montana.
