Let me help out a bit.

I can't think of ANY carriers that write both workers comp, and health insurance.

And unless your workers comp premium is in excess of $50,000, no one is going to be interested in writing it 'stand alone' - which means, you'll have to put it with the same company that does your general liability.

Depending on what TYPE of small business you are (Bakery? Construction? Vetrinarian's office?), and how "small" you are (it's in the eye of the beholder), and what kind of claims history you have (meaning, prior insurance experience, not just "I have no claims") and for how long, is going to decide which companies in YOUR state would be giving you the most competitive rates.

It's really not a 'do it yourself' kinda thing. You're probably best off contacting a few direct agents, AND a few independent agents, who can explain things in more detail, that's relevant for YOUR business type.

Your best bet is to check with an independent agent in your area. They have access to a number of different companies and can shop it around for you. All the previously mentioned companies here specialize in personal lines insurance. Your rate is going to depend on a number of different factors including where the business is located, years of experience, how much coverage you need etc.

Check one of the Small Business Development Centers (also called SBDC - there are several across the state, including one at B.C., linked below) to see if they have the information available for you. They help businesses get started in the Commonwealth, using both state and federal funding, so they have a lot of data and information for starting and managing many different kinds of businesses.

The SBDC probably has "property and casualty" (a.k.a. "business")insurance people on staff or associated instructors who would be willing to discuss the different kinds of insurance you would need and the associated costs.

Liquor licenses are awarded by the town where the sports bar would be located. Costs may vary by town, but I know that there is usually an application fee and also an annual fee (fees are a big source of "non-tax" revenue).
