Yes. It generally depends on the driver's record. I've seen drivers dropped because they didn't meet the insurer's standards.

C is most correct...

B can also be correct in some circumstances... But not usually for group/commercial insurance. RBRVS is not as widely used now as it one was... The fee for service schedule may be based on RBRVS.

And... If you're taking a test that asks this question... Its most certainly in a book you have in your possession...

Resource based relative value scale is the most popular however; there are still some traditional fee-for-service arranagements still out there.

The most common RVS is using the HIAA scale. Most companies are currently paying at the 80th percentile.
