The problem comes when someone who can't afford anything beyond the low tier gets cancer and exhausts his coverage. He then goes to the emergency room and gets treated (badly). These costs are then passed to those with insurance, or to the taxpayer.

We need to get every American into the health insurance pool. The money you pay in when you are in your 20's and 30's pay for the care you will need after age 50. In the event you get cancer at age 20, you are covered. Right now, people in their 20's can chance going without insurance, and pick it up when they are older and more likely to get sick. That is the worst possible scenario.

The current system of employer provided insurance is terrible. No one knows how much their insurance is worth. There is no incentive to save money, other than co-payments. There is no incentive for doctors, hospitals, or drug companies to be competitive on price. Then you lose your job and your insurance is gone, and any preexisting condition means you can forget buying an individual policy.

We might be better off giving people medical savings accounts to pay for routine care like checkups or visits for the flu. These could be subsidized for the poor. The money you didn't spend in one year would carry over into the next. Think of it as car insurance. You don't put in an insurance claim to get an oil change in your car. You also would probably go to the oil change shop that had a lower advertised price. You would have the same incentive to shop for doctor's costs, treatment costs, etc., as you do for your car. Everyone would then pay into a separate plan that would kick in if you get a major illness or chronic condition, or if your spending went above a set percentage of your income.
